The purpose of this charter team is to develop a strategic plan that will focus on the necessary components of a comprehensive
continuum of care for hospital to home community integration. This plan will connect to and be a part of the Mental Health
system's 5 year strategic plan as coordinated with the Governor's MH Planning Council. The charter for this team was written utilizing
the recommendations of the committees listed above, available data and resources from other states. The strategic plan will focus on "what works"
to design a collectively desired future for the following components:
- Outreach and Access to Community Based Services
- Assessment/Liaison/Discharge Process
- Crisis Prevention and Intervention
- Evaluation of Funding Streams
