The hospital and home core team and transformation subcommittee have existed as
two separate groups. In January 2011, steps were taken to combine the two groups
who already had similar missions, to become one group moving forward.
The newly formed hospital and home transformation team will gather strategic actions
being implemented by SRS and GMHSPC subcommittees, establish a common system to
categorize each strategic action in order to identify commonalities, prioritize
actions based on agreed upon criteria, and evaluate progress towards the vision
by analyzing and synthesizing data to measure consumer outcomes previously established.
